Improper documentation and coding for Evaluation and Management office visit services has been a major target for audit by CMS. In 2010, the Office of the Inspector General found that 42% of claims for Medicare E&M services were incorrectly coded resulting in $6.7B in inappropriate payments.

Karen Scott will discuss both current and upcoming changes in rules for assigning the proper evaluation and management CPT codes and documenting for services rendered.
