This seminar will help you document and code accurately to reflect services rendered, support medical necessity, and relate performance measures.

Transition Care Management (TCM) is becoming more important as a process, not only to decrease ED visits and hospital readmits, but also to increase patient loyalty and revenue opportunities for Medicare and now also for commercial payers. The necessary elements for performing/billing TCM will be discussed.
