Appropriate use of a 25 modifier has been causing confusion and presenting coding compliance issues since its inception. Use of this modifier is being heavily audited, and payers are beginning to require providers to submit documentation much more frequently than in the past.

Join us for a detailed discussion of proper use. This session is very appropriate for clinicians as well as for coders.

Chart Auditing for Coding and Documentation in 2025

This interactive session is designed for coders and will focus on techniques for auditing clinical documentation to justify appropriate assignment of ICD-10 CM codes with attention to those that trigger HCC for risk.

We will also review proper documentation for E/M.Objectives include:
• Assisting with the implementation of your own internal audit program
• Providing exercises for hands on auditing experience
• Guiding you with effective feedback to your physician and staff

Be sure to bring your current CPT-4 and ICD-10 CM manuals with you as well as a highlighter and sticky notes.  Feel free to also bring your questions or problems.

This class will review the coding processes, rules, sequencing guidelines, and regulations relating to the musculoskeletal system.   Fracture codes as well as codes for disorders, anomalies, conditions and injuries will be covered. Have your manuals ready for some coding exercises 

Join us to ensure documentation and coding pieces come together to appropriately relate the service rendered and report performance.


A few topics to be discussed are:
• Coding external causes
• Coding treatment after a prior surgical loss
• Bundled payments
• Coding E/M and procedures on the same day
• Fractures: delayed healing, nonunion, malunion, sequela
