DEFENSE AGAINST DOWNCODING: Reporting E & M Levels 4 and 5 in 2026 July 21 Presented by: Karen Scott, MEd, RHIA, CCS-P, CPC, FAHIMA Physicians: We want to help you continue to be paid appropriately for all of the work that you do to provide the best care for your patients. New tactics are being used by payers to lower the E&M levels that you report and pay at that level without even considering additional documentation. Karen will review in detail and lead a discussion around what constitutes E&M levels 4 and 5 and how you can best document to protect your claims coding based on medical decision making. Time documentation will also be discussed.
